$145 per 60 minute session for Dr. Perrymore

Insurance
I choose to only work with a Blue Cross Blue Shield PPO plans and work with other insurance companies through out-of-network benefits.  For specific insurance inquiries about using out-of-network insurance benefits please call your provider for information regarding your insurance plan's out-of-network benefits program and reimbursement schedule.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 48 hours in advance, you will be required to pay the full cost of the session.
